Medication Procedure

A reminder please that as of September 2025 we require parents to complete the attached medicine form for any form of medication to be issued in school. Any medication which we currently have on site that does not have an accompanying medication form will be disposed of.

What should I do if my child requires medication?

If your son/daughter requires any form of medication to be administered during the school day a ‘request for administering medication’ form must be completed and returned to the school. This will have the details of medication names, dosage, expiry dates, times to be administered and circumstances which would be appropriate to administer medication. There should be one form per medication. For example, if your son/daughter has an allergy and needs an antihistamine and adrenaline auto-injector, you should complete two forms.

What about if my child already has medication in school?

If your son/daughter has medication currently in school, please complete the attached form as soon as possible. It is essential that the school has an up to date medication form for all students who require medication during the school day. A link to the form will also be available on the school website. If you are unable to access this form electronically then please email man-info@cset.co.uk and we can send a letter home with your child.

Long term medicine form

Short term medicine form

I would like to keep some medication in school for my child, what should I do?

If your son/daughter does not currently have medication in school but requires some, please complete the documents linked above and we will confirm an appropriate time for you to bring in the medication. Should medication be required to be administered on short notice you will be asked to complete the form prior to us being able to store the medication in school.

How should the medication be presented at school?

Medication should be brought into school in their original packaging, clearly labelled with the name of the student, the name and dose of the drug, the frequency of administration, any likely side effects, and the expiry date. The exception to this is insulin, which must still be in date but will generally be available to schools inside an insulin pump rather than its original packaging. Parents are responsible for ensuring that this information is provided.

If your child has an inhaler, antihistamine or autoinjector (e.g Epipen/emerade), we suggest they carry this on their person and keep a spare in reception in case of emergencies.

What happens to the medication if it expires or if my child leaves the school?

If your child leaves Mangotsfield School and medication needs to be returned, we will need to sign back the medication to the parent/carer. This also applies with expired medication.
